MEDIALLY BASED SKIN FLAP

The classical incisions for mandibulectomy and maxillectomy are medial with laterally based flaps

They function very well in most cases, but when it comes to the huge “African size” tumors there is a risk that the medial part of the flap will necrotize due to embarrassed blood supply caused by the extensive dissection necessary to remove a large tumor. Then you will face a major catastrophe as the following pictures show

With large tumors a lateral incision with a medially based flap is therefore preferable

After removal of the tumor the skin surplus can easily be excised ……

…and if necrosis of the flap occurs the problem is easily solved and the complication will not cause any further disfigurement of the face.
